Remaining shelf life was first asked about in April 2024. It is captured in the WMS but not exposed; the system owner (Nils Ackermann) was asked twice, in 2024 and 2025. Marco Rossi’s June 2026 written position: for the next number to be different, that field has to be available — it is not a RELEX deliverable, and without it chain-wide will reduce waste by less than the pilot suggests.

Three weekly “no errors” reports are evidence of job completion only — and the data does not reconcile
The weekly updates of 12 January, 9 February and 9 March 2026 each report “Nightly article extract completed, no errors”. In June 2026 Nadia Haddad put the extract totals next to the till numbers for January–March: “They do not reconcile. The extract is lower… Nobody has ever checked what was in it.” The 3.4% figure taken to the board in April is derived from those totals.

A joint engineering session on 4 June 2025 produced a permanent-fix plan dated 15 September 2025, gated on Acme’s quarterly change window. The archive holds no completion record; the January 2026 incident log still counts fourteen failures (eleven silent), and Meridian was told in February to build assuming “roughly fortnightly failures”.

A three-wave rollout was planned as if Acme had agreed it — nobody had
Marco Rossi’s June 2025 three-wave proposal was never answered by Acme. The plan was built on it anyway (“discussed verbally in the June steering and nobody pushed back. I would treat it as agreed.”), with Nadia Haddad’s challenge on record since August. Lena Fischer formally rejected it in November 2025 and Marco admitted: “it was not minuted as a decision, and we built the plan on it. That is our error.” The recorded decision is two deliveries.

A “40 stores” figure circulated in May 2025 after the cohort had been 25 since September 2024
Marco Rossi’s 20 May 2025 email to Robert Kahn described the 90-day headline numbers as drawn from “the pilot cohort of 40 stores”. The cohort was reduced to 25 in September 2024 — so the statement was wrong when written, not overtaken later. Kahn had seen both numbers within a month; the correction came the next day, and the stale figure survived in the kickoff material and a slide until a folder note was added in July 2025.

Nadia Haddad proposed the approximation on 4 September 2025; Priya Nair answered “Proceed with your approximation” a week later. When Ana Duarte audited the thread in October she could not find an agreement anywhere; the last word on the thread is “We proceeded. Nobody objected.” The approximation biases slow movers optimistic — on the slowest tenth, possibly two days.

Sofia Almeida is the only person at Acme who understands the fresh categories; she flagged it herself in the demo. The point sat in three risk registers “in slightly different words” — Charlotte Meyer’s verdict: “Then it is in none.” As of June 2026 she still spends about a day and a half a week ordering by hand on the slow-moving lines.

After the holiday-coverage failure Ana Duarte committed to “a named second person on the runbook permanently by the end of next week”. The archive never records who that was; the fresh cutover in December leaned on the same single-point arrangement again.

Tomas Lindholm withdrew his own cutover recommendation on 18 March 2025: “A file can be the right size and still be the wrong file.” The recommendation still sat in the cutover pack, the runbook and the hypercare plan — written once, copied, and never removed from any of the three.
